Background Checks
All club board members, coaches and assistants must annually complete the Oregon Youth Soccer Association volunteer disclosure form. This form is used to perform a multi-state background check that assures the safety of the children in our care.

Practice Field Requests
Coaches can request a practice field by filling out the West Hills club field request form. Complete and return this form to the club field coordinator.
